Forms

Before you start at K+S Potash Canada, there are several forms that you will need to fill out. They are all found below. Please ensure that you have the following forms completed and with you on your first day. If you have any questions do not worry, there will be time allotted during the first day to review the forms and ask any questions.

Please note:

• Your KS Number (Employee Number) will be provided on your first day. If needed on the forms, please leave blank until your start date.

• Some web browsers are not enabled to open PDF forms in a viewer window. If the form does not open, select “Save as” to save the file on your system. Navigate to the folder where you saved it, and open the PDF document from there.

• Spending Account Declaration (not applicable for students)

  • Health Care Spending Account Summary

  • Taxable Spending Account Summary

• Manulife Pension Enrollment (Registered Pension Plan) (not applicable for students or temporary employees)

• TD1

• TD1-SK

  Please have your banking information readily available with you on your first day. This will be required to be entered into the payroll system by you upon your start.

Optional Forms

• Retirement Savings Plan Enrollment (RRSP) (not applicable for students or temporary employees)

• Retirement Savings Plan _ Spousal (Spousal RRSP) (not applicable for students or temporary employees)

• Tax-Free Savings Account (TFSA) (not applicable for students or temporary employees)
